Care Coordination
Registration for Services
- A full intake qualifies HIV+ clients to receive services at Shanti, the San Francisco AIDS Foundation and the UCSF AIDS Health Project.
- A limited intake for service only from Shanti Activities.
- Re-registration when current registration has expired. Registrations must be updated yearly and can often be completed by telephone.
Services from a Shanti Peer Support Volunteer
- Registration for a Peer Support Volunteer who can provide practical assistance (such as light housekeeping, grocery shopping, and errands) and emotional support (such as supportive conversation, companionship, encouragement for treatment issues). Care Coordinators assist clients in completing the application process and matching a client with a peer support volunteer.
- Regular follow up ensures that the client-volunteer match is serving a client's need.
Information and Referral
- Care Coordinators provide information about HIV/AIDS issues, the services available at Shanti and our collaborative partners, services at other San Francisco agencies, and more. Information can be obtained during drop-in hours or in a scheduled appointment.
Transportation
- Care Coordinators can provide a referral for same day van service from the 4th Floor Drop-in Center to home during the hours of 11:00 a.m.-1:00 p.m., Tuesday through Friday.
Care Coordination services are provided on a drop-in basis:
Tuesday through Thursday 9:30 a.m.-12:30 p.m.
Care Coordination Services are provided by appointment only:
Monday 1 p.m.-5 p.m.
Tuesday through Thursday 2:30 p.m.-5 p.m.
Friday 9 a.m.-5 p.m.

Serving over 1,000 clients annually, care coordinators provide intake services to programs at Shanti and other collaboration partner agencies, information and referral for city-wide services, and support to clients and their peer support volunteers.
