HIV PROGRAMS CLIENT SERVICES

We offer the following services for clients in our HIV Programs.

Medical Case Management, Care Navigation, and Client Advocacy

Urgent needs assessment and referrals to appropriate vital support services; treatment adherence support and medical appointment compliance; harm reduction counseling; emotional and practical support; ongoing care coordination; and matching and support of client/volunteer matches.

Individual Health Counseling

Support of clients in developing health-protective routines and addressing the psychological, social, and biological cofactors that can impact overall health and well-being.

Peer Support Volunteers

Shanti Volunteer Caregivers are trained in the Shanti Model of Peer Support™, with trainings offered at least five times per year. Clients are then matched with a volunteer, who provides an average of three hours of emotional and/or practical support per week, based on the client’s needs.
